24/7 Community Recovery | Mental Health Support At Home

Northern Healthcare now offer community-based mental health support to individuals at home across the North West, Yorkshire and Cumbria. With extensive experience supporting individuals with a range of mental health difficulties in our shared residential services, our mental health support teams were approached by adult health and social care services to offer the same high level of tailored mental health support to individuals in their own homes.

Utilising the ‘Northern Healthcare Supported Living Model’ allows the people we support to live as independently as possible. The quality of the support is the same as that provided to an individual living at a Northern Healthcare shared residential service.

What is included

Tailored support plans

The support provided is based on the individual's needs; it will include around-the-clock support from a dedicated team and may also include clinical recovery-based therapies with input from a Registered Mental Health Nurse and Occupational Therapist.

Our teams will help with medication management and identifying and accessing additional therapies as required.

Support with shopping, nutrition, cooking and menu-planning and general health and wellbeing.

Accessing education, employment and volunteering opportunities. Signposting appropriate social and leisure activities in the local community.

Managing bills and money & identifying and claiming benefits.

As well as offering practical support our team provide emotional support, companionship and encouragement.

Our team are trained by specialist with vast experience to understand how to support individuals with complex mental health problems and are always on-hand for the individuals we support.

We treat everybody as an individual empowering them to lead safe, purposeful and happy lives. Our support quality and safety standards are supported by comprehensive risk management and rigorous internal and external clinical governance processes.

Our teams include a Registered Mental Health Nurse, a Registered Learning Disability Nurse and highly trained Support Workers with the ability to facilitate access to other specialist professions as required, for example Cognitive Behavioural Therapy.

At Northern Healthcare our teams provide support for people living with a variety of mental health conditions, learning difficulties and autistic spectrum disorders. Some of the people we support may have more than one diagnosis, but every person is treated as an individual. Whilst personal history and diagnosis can help our teams understand and support individuals, we are dedicated to seeing the person and not their condition. All of our support packages are tailored to the individual’s needs and a core team will be assigned to the individual based upon their needs.

We work closely with local authorities and health care professionals to deliver comprehensive support packages based on an individual’s support requirements and goals for recovery.

FAQs

How has the service responded to COVID-19?

To keep the people we support and our team members safe, our team are required to wear PPE, as a minimum this is a face covering. We are closely monitoring the situation and are prepared to scale-up requirements as and when it is needed.

Packages can vary depending on need, our teams can support people on an hourly basis or up to 24/7. Support packages are consistently reviewed and tailored to each individuals changing needs.

We can support an individual with daily-living tasks such as rent/money management. However, unlike our residential services, housing is completely independent and is often privately owned/rented or funded independently through the individual’s benefits or earnings.

We accept referrals from professionals working in adult health and social care, this may be a Social Worker or Case Manager for example. To submit a referral please get in touch or visit our ‘Support Networks’ pages for more information.

We work collaboratively with families and friends to build support networks around the individual in our care. These support networks are vital in helping the people we support to meet their goals, reduce the risk of relapse and to optimise and support their recovery journey.

Building support networks

For us to help the people we support it is vitally important to us to form long-lasting relationships with the people they know and trust. We support commissioners and referrers to find the right service for the individual in their care. Building a network of support around an individual is crucial in aiding their development – families, friends and the wider community all have an important role to play.
