Our 24/7 supported living services across Yorkshire empower individuals to lead happy fulfilling lives in a safe environment

Our specialist teams provide support for people living with a mental health diagnosis, autism, learning disability and complex needs. People may come to us for support following a hospital stay, our teams offer discharge support for long-term mental health care enabling individuals to regain their independence, and hospital avoidance pathways for individuals who may require short-term or ‘crisis intervention’ support before returning home.

Support can be provided in an individual’s home or at one of our shared supported accommodation services located across Yorkshire in Shipley, Pudsey, Wakefield and Hull. All of our services and support at home packages offer the same level of clinical mental health support 24/7. Services are renovated to a very high standard and we support individuals to maintain tenancies and promote greater independence in a safe and homely environment.

We work closely with adult health and social care professionals such as Safeguarding, NHS Trusts, Councils, Community Mental Health Teams (CMHT’s), Wellbeing and Pathway Teams, Early Intervention Professionals and Forensic Teams, to ensure Northern Healthcare is the next best step for individuals in their care.

We support individuals on their journey to independent living by:

  • Developing support plans based on their strengths, choices and future goals
  • Building relationships with their wider support networks including, their families, carers advocates and other people who are important to them
  • Helping them to maintain their own tenancy agreement with an independent registered housing association
  • Making sure that individuals understand their rights and choices
  • Providing person-centred practical support and occupational therapy to help with everyday living and supporting mental and physical wellbeing
  • Helping individuals to build relationships and access their local community to build independence at their own pace
