Discharge to Assess: Temporary Supported Living

Our Discharge to Assess (D2A) service provides a structured, clinically-informed step-down from hospital care (Psychiatric Intensive Care Units and Acute Wards) to temporary supported living. This enables earlier discharge into safe, 24-hour on-site support within temporary accommodation, typically for a 12 week period, ensuring individuals continue receiving care within reach of their local community mental health team.

    Our Discharge to Assess service offers a safe and supportive environment. It provides short-term stability while giving individuals the space and guidance to explore what the next stage of their life could look like. With a structured approach, we work alongside individuals to build essential life skills, increase confidence, and support individuals to move forward when the time is right. We work collaboratively with Integrated Care Partnerships (ICPs) to improve outcomes for individuals with mental health diagnoses by:

    • Enhancing support and streamlining care transitions
    • Reducing hospital admissions
    • Optimising discharge planning

    This coordinated approach ensures that care remains consistent, compassionate, and person-centred throughout the recovery process.

    Features of the Discharge to Assess Pathway

    Our service supports individuals who are ready to leave hospital but need additional time to organise stable housing or access essential support services. It is also designed for those who may require short-term or crisis intervention support before returning home. Many benefit from a structured and supportive step-down environment that provides stability, guidance, and continuity of care as part of their recovery journey. As well as the emotional and mental health support that individuals often need following a long-term stay in an inpatient setting, there are lots of practical support elements that we often take for granted. The goal is always to safely return to independent living, our services provide individuals with safe spaces, the support and the time to do so.
